Colorado Camps Medical Forms Guide

Because of the laws in Colorado, our camps are considered childcare facilities. In order to comply with all state regulations, we do things a little differently than other camps around the country. The guide linked above will give a complete explanation of how and what you must do to comply with state and local laws.

BSA Colorado Camp Medical Forms – Required for ALL youth participants

All youth participants who attend a camp in the Pathway to the Rockies Council are required to fill out this form. Please read the instructions of this form as they will help you better understand the purpose of this document as well as other requirements.

This document is a combined document of the following required documents:

  • Annual Health & Medical Record
  • Copy of Health Insurance
  • Colorado Certificate of Immunization
  • Colorado Addendum which includes:
    • Parent / Guardian Contact Information
    • Authorized Persons Information
    • Sunscreen Permission

NOTE: Colorado does not allow chiropractors to sign off on any medical documents for camp.

Annual Health & Medical Record

Parts A, B, & C are required for all youth & adults attending camp, regardless of how long they intend to spend at camp. If you an adult, please click the link above to complete this document. If you wish to complete this for a Scout / youth participant, please download the BSA Colorado Camp Medical Forms – Required for ALL youth participants

Do not fill out this form until you have completely reviewed the Colorado Camps Medical Forms Guide. Because our camp is considered a childcare facility, this document must be filled out in a specific way. Not filling out this form correctly may void registration for that individual if correction(s) cannot be made prior to camp.

Permission for Medication Administration

Required for all youth who will have prescription AND/OR over-the-counter medications (including vitamins).

Youth are not allowed to keep any medication on their person while at camp. This includes but is not limited to over-the-counter medications such as ibuprofen, Tylenol, vitamins, essential oils, etc. If you or a parent has questions about what requires this form, please call us.

Contract to Carry

Required for youth who must carry medication of a time-sensitive nature, such as an EpiPen or rescue inhaler.

While most medication is required to be dispensed by our camp health officer, some are required to be used in an emergency situation. In these situations, youth participants are required to have this document in their medical forms.

Dietary Restrictions

Updated 3/4/2022

For individuals who have allergies or dietary restrictions. Please fill this form out completely and submit it along with your other medical forms. This will give us a heads up as to how to handle special meals, especially when it comes to ordering.

If this document does not arrive a minimum of 14 days prior to the first day of your camp session, substitutions may not be available for meals.
